外耳道内推注曲安奈德益康唑乳膏治疗真菌性外耳道炎的

疗效观察

【摘要】目的：探讨外耳道内推注曲安奈德益康唑乳膏治疗真菌性外耳道炎的临床效果。方

法：选取 2021.6.1-2022.6.1 我院耳鼻喉科收治的 92 例（92 耳）真菌性外耳道炎患者，根据
治疗方法的不同分为对照组和病例组，各 46 例。所有患者首先于耳内镜下清洁外耳道，对

照组嘱患者回家后自行涂药 2 次/日，14 天后复查。病例组由医师向外耳道内推注曲安奈德
益康唑乳膏并吸除多余膏体，每 3 天复诊 1 次，共治疗 5 次。比较两组的临床疗效、不良反

应。随访 3 个月，比较两组复发率。结果：对照组总有效率 38/46（82.61%），病例组总有

效率 44/46（95.65%），病例组总有效率高于对照组，P=0.044（小于 0.05），差异有统计学
意义。结论：向外耳道内推注曲安奈德益康唑乳膏治疗真菌性外耳道炎疗效确切、操作简单，

值得临床推广。

[Abstract] Objective：To investigate the clinical effect of triamcinolone acetonide and econazole
cream in the treatment of fungal otitis externa. Methods：92 patients (92 ears) with fungal external

otitis were selected from the department of otolaryngology of our hospital from January 2021.6.1
to January 2022.6.1. They were divided into control group and case group according to different
treatment methods, with 46 cases each. All patients first cleaned the external auditory canal under

the ear endoscope, and the control group instructed the patients to apply medicine twice a day after
returning home, and then rechecked 14 days later. In the case group, the physician injected
triamcinolone acetonide and econazole cream into the external ear canal and sucked off the excess
cream. The patient was revisited once every 3 days, with a total of 5 times of treatment. The clinical

efficacy and adverse reactions of the two groups were compared. Follow up for 3 months and
compare the recurrence rate of the two groups. Results：The total effective rate of the control group
was 38/46 (82.61%), and the total effective rate of the case group was 44/46 (95.65%). The total

effective rate of the case group was higher than that of the control group, P=0.044 (less than 0.05),
and the difference was statistically significant. Conclusion：Injecting triamcinolone acetonide and
econazole cream into the external auditory canal in the treatment of fungal otitis externa has definite

curative effect and simple operation, which is worthy of clinical promotion.

真菌性外耳道炎是耳鼻喉科常见的疾病之一，多数由于使用不洁物品挖耳、过度挖耳、
免疫力降低、长期应用抗生素滴耳剂等原因造成 ，主要症状为耳痒、耳闷、耳鸣、流脓、
