                     鼻窦 CT 在降低鼻窦炎手术患者术中筛前动脉损伤风险中

                   【摘要】目的:探讨鼻窦 CT 扫描及重建在明确筛前动脉与颅底位置关系和降低鼻窦炎手术
                   患者术中筛前动脉损伤风险中的应用价值。方法:选取 2021 年 8 月~2022 年 8 月我科收治

                   入院的慢性鼻窦炎患者 60 例(120 侧),均行鼻窦 CT 扫描及重建,对其临床资料进行回顾
                   性分析。采用 Lannoy 分型法并依据筛前动脉与颅底的位置关系将其分为Ⅰ~Ⅲ型筛前动脉,

                   计算筛前动脉悬空率(Ⅲ型筛前动脉发生率); 依据筛板外侧板相对于筛顶的深度进行分型
                   (Keros 分型)并判断颅底情况,采用不同 CT 影像及辅助定位方法明确眶上筛房(SOEC),

                   分析筛前动脉与 Keros 分型、SOEC 的相关性。结果:鼻窦 CT 扫描及重建结果显示,Ⅰ型筛
                   前动脉占 43.33%(52/120), Ⅱ型占 18.33%(22/120),Ⅲ型(筛前动脉悬空率)占 38.34%

                   (46/120)。 Keros 分型中Ⅰ型占 52.50%(63/120), 筛前动脉悬空 11 侧,悬空率为 17.46%

                   (11/63); Ⅱ型占 38.33%(46/120),筛前动脉悬空 24 侧,悬空率为 52.17%(24/46); Ⅲ型
                   占 9.17%(11/120),筛前动脉悬空 11 侧,悬空率 100.00%(11/11)。 Spearson 相关分析结果

                   显示 Keros 分型与筛前动脉悬空呈中度正相关(r=0.512,P<0.001)。120 侧中含 SOEC21 侧,
                   占 17.50%(21/120);筛前动脉悬空 14 侧,占 66.67%(14/21);不含 SOEC99 侧,占 82.50%

                   (99/120);筛前动脉悬空 32 侧,占 32.32%(32/99)。含 SOEC 组筛前动脉悬空率明显高于

                   不含 SOEC 组(χ =8.644,P=0.003<0.05)。结论:筛前动脉悬空与 Keros 分型升高、存在 SOEC
                   密切相关,术前行鼻窦 CT 可了解筛前动脉与颅底位置关系,进而减少术中筛前动脉损伤。

                     Application of CT to reduce the risk of intraoperative anterior ethmoidal artery injury in
                                                   patients with sinusitis
                   [Abstract] Objective: To investigate the application value of CT scanning and reconstruction of

                   sinuses in clarifying the relationship between anterior ethmoid artery and skull base and reducing
                   the risk of intraoperative injury of anterior ethmoid artery in patients with sinusitis. Methods: Sixty
                   patients (120 sides) with chronic sinusitis admitted to our department from August 2021 to August

                   2022 were selected, all of whom underwent CT scanning and reconstruction of sinuses, and their
                   clinical data were retrospectively analyzed. The anterior ethmoid artery was divided into type I ~ Ⅲ
                   anterior ethmoid artery according to the position relationship between the anterior ethmoid artery
                   and the skull base by Lannoy classification method, and the suspended rate of the anterior ethmoid
                   artery (type Ⅲ incidence rate of the anterior ethmoid artery) was calculated. Keros classification

                   was performed according to the depth of the lateral ethmoid plate relative to the ethmoid roof, and
