耐碳青酶烯类肺炎克雷伯菌多重耐药的危险因素分析及
预测模型的构建

【摘要】目的:探究耐碳青霉烯类肺炎克雷伯菌(Klebsiella pneumoniae)多重耐药的危险因
素。方法:本研究通过收集 2017 年 1 月 1 日-2021 年 12 月 31 日在我院呼吸科住院的 196 例
肺炎克雷伯菌感染的住院患者作为研究对象,分耐碳青酶烯类多重耐药组(n=48)和肺炎克
雷伯菌非多重耐药组(n=148)。通过 Lasso 分析筛选出耐碳青酶烯类肺炎克雷伯菌多重耐
药的危险因素,并通过 Logstic 多因素回归分析计算各个危险因素的 P 值,之后将危险因素
用于构建耐碳青酶烯类肺炎克雷伯菌多重耐药的列线图预测模型,使用 ROC 曲线及校准图
以及内部随机数据验证的方式对预测模型进行验证。结果:耐碳青酶烯类肺炎克雷伯菌多重
耐药的危险因素有:住院天数、30 天内住过 ICU、降钙素原高、格拉斯评分低、合并真菌感
染、应用碳青霉烯类药物、应用免疫抑制、应用抑酸药物、应用糖皮质激素、合并肾功能损
伤、合并高血压、合并脑血管。计算出预测模型的 C 指数为 0.970,说明模型具有较高的预
测准确度,最后对预测模型进行内部验证,显示预测模型有较高的准确度。结论:住院天数、
30 天内住过 ICU、降钙素原高、格拉斯评分低、合并真菌感染、应用碳青霉烯类药物、应用
免疫抑制、应用抑酸药物、应用糖皮质激素、合并肾功能损伤、合并高血压、合并脑血管是
耐碳青霉烯类肺炎克雷伯菌多重耐药的危险因素,据此构建的预测模型有较好的预测价值,
可为临床医护人员制定个体化防治策略提供参考。

Analysis of risk factors for multidrug-resistant Klebsiella pneumoniae resistant to
carbapenems and build a predictive model

[Abstract] Objective: To analysis the risk factors of multidrug-resistant Klebsiella pneumoniae,
which is resistant to carbapenems. Method: This study collected 196 hospitalized patients with
Klebsiella pneumoniae who were hospitalized in the respiratory department of our hospital from
January 1, 2017 to December 31, 2021, as the research subjects. They were divided into a
carbapenem resistant multidrug-resistant group (n=48) and a non multidrug-resistant group (n=148).
The risk factors for multidrug resistance of carbapenem resistant Klebsiella pneumoniae were
screened through Lasso analysis, and the P values of each risk factor were calculated through
Logstic multiple factor regression analysis. The risk factors were then used to construct a column
chart prediction model for multidrug resistance of carbapenem resistant Klebsiella pneumoniae, and
the prediction model was validated using ROC curves, calibration charts, and internal data
validation. Result: The risk factors for multidrug resistance of carbapenem resistant Klebsiella
