真实世界中莲必治注射液疑似致肾功能异常的影响因素

【摘要】目的：分析真实世界中莲必治注射液疑似致肾功能异常患者的临床特征，探索使用

莲必治注射液后出现肾功能异常的危险因素。方法：对我院 2019 年 191 例使用莲必治注射
液的患者进行回顾性分析，根据应用莲必治注射液后肾功能是否出现异常变化分为用药后异

常变化组和无异常变化组，使用 Logistic 回归模型分析应用莲必治注射液引起肾功能异常的

可疑因素。结果：用药前肌酐异常，合并使用万古霉素、呋塞米、头孢噻肟钠舒巴坦可能是

发生肾功能异常的影响因素（P＜0.05） 。 结论：在应用莲必治注射液前应检查患者的肌酐水

平，且谨慎与万古霉素等影响肾功能的药物合用。

Real World Study on the influencing factors of Suspected Abnormal Renal Function in

Patients Treated with Lianbizhi Injection

Abstract: Objective To analyze the clinical features of patients with abnormal renal function

Suspicion were caused by Lianbizhi Injection in real world，and to explore the risk factors of

abnormal renal function after the use of Lianbizhi injection. Methods A retrospective study among
the 191 cases who treated by Lianbizhi Injection in 2019，and the patients were divided into two

groups according to whether the abnormal changes of renal function after the application of

Lianbizhi injection. Influencing factors were screened with Logistic regression analysis. Results It

was found that the use of Lianbizhi injection combined with abnormal creatinine or the combination

of vancomycin, furosemide, cefotaxime/ sulbactam were the influencing factors of the abnormal

changes of renal function. The result was statistically significant (P＜0.05). Conclusion The

creatinine level of patients should be checked before the use of Lianbi injection, and it should be

carefully combined with vancomycin and other drugs that affect renal function.

莲必治注射液的主要成分为亚硫酸氢钠穿心莲内酯，具有清热解毒、抗菌消炎的功效，

在临床上被用于细菌性痢疾、肺炎和急性扁桃体炎 。随着莲必治注射液的广泛应用，其药
品不良反应的报道也不断出现。2005 年国家药品不良反应监测中心发布《药品不良反应信

息通报》（第 8 期）通报了莲必治注射液的不良反应，其中急性肾损害有 17 例 。2006 年
