                   纤溶酶原激活物抑制剂-1 在脓毒症致 DIC 的早期诊断价值

                   【摘要】目的:探讨纤溶酶原激活物抑制剂-1(Plasminogen activator inhibitortype-1,PAI-1)
                   在脓毒症所致弥散性血管内凝血(Disseminated intrascular coagulation,DIC)中的早期诊断价

                   值。方法:纳入 85 例临床诊断为脓毒症的患者的临床病例资料,并根 ISTH 诊断标准分为
                   DIC 组和非 DIC 组,采用酶免法检测其血 PAI-1 值,评估其 DIC 早期诊断中的价值。结果:

                   在 DIC 患者中,其血 PAI-1 值在第 1、3、7 天较非 DIC 组均有显著升高(P<0.05);受试
                   者工作曲线(ROC)分析提示 PAI-1 在患者中第 1、3、7 天的 AUC 面积分别为 0.746(95%CI

                   0.553~0.875)、 0.913(95%CI 0.831~0.965)、 0.884(95%CI 0.712~0.953);皮尔逊相关性分析

                   则发现血 PAI-1 值与 SOFA 评分、APACHEⅡ评分存在显著相关性(P<0.05)。 结论:血 PAI-
                   1 值可用于脓毒症所致 DIC 的早期诊断,为临床早期干预和治疗提供最佳时机。

                       The Value of F-lysogen-activator Inhibitor-1 in The Early Diagnosis of DIC in Sepsis

                     [Abstract]  Objective:  To  explore  the  early  diagnostic  value  of  plasminogen  activator
                   inhibitortype-1  (PAI-1)  in  disseminated  intravascular  coagulation  (DIC)  in  sepsis.  Methods:
                   Eighty-five patients with sepsis were divided into DIC group and non-DIC group according to the

                   ISTH, and their blood PAI-1 value was detected by enzyme immunoassay to evaluate its value in
                   early diagnosis of DIC. Results: The level of PAI-1 in DIC patients was significantly higher than
                   that in non-DIC group on the 137th day (P < 0.05); The AUC area of PAI-1 on day 137th was 0.746
                   (95% CI 0.553 ~ 0.875) 0.913 (95% CI 0.831 ~ 0.965) 0.884 (95% CI 26.43); Pearson correlation

                   analysis showed that there was significant correlation between serum PAI-1 and SOFA score and
                   APACHE ⅱ score (P < 0.05) . Conclusion: The value of blood PAI-1 can be used in the early
                   diagnosis of DIC caused by sepsis and provide the best time for early intervention and treatment.

                       脓毒症是指机体对感染反应失调所致的器官功能障碍,且是感染引起死亡的主要原因 。

                   弥漫性血管内凝血(Disseminated intrascular coagulation,DIC)涉及广泛和持续的凝血激活,
                   最终导致全身小血管出现弥漫性微血栓。先前的研究表明,约有 20-40%的脓毒症可继发 DIC,

                   并进一步导致组织灌注不足和多器官功能衰竭 。因此,及早识别脓毒症诱发的 DIC 对其治
                   疗极为关键。中国指南上推荐使用国际血栓与止血学会诊断标准(ISTH-DIC)、中国 DIC 诊

                   断积分系统(CDSS-DIC)评分系统诊断 DIC,但它们都无法在疾病早期准确识别脓毒症所
                   诱发 DIC。
