                                        HPV16感染患者临床特点分析

                   【摘要】目的:探讨感染高危型人乳头瘤病毒(HPV)的临床特点。方法:选择2013-01至2021-
                   12北京京煤集团总医院行HPV分子检测中HPV16阳性患者542例,分析患者合并感染其他

                   HPV情况、不同年龄段感染情况及HPV16转阴情况。结果:(1)542例中,单一阳性330例,占
                   60.9%;合并其他高危感染173例,占31.9%;合并低危感染39例,占7.2%;合并其他高危组

                   宫颈活检CIN Ⅱ及以上患病率(44.2%)高于合并低危组(21.4%, χ =4.880, P=0.027)。(2)≤29岁、
                   30~39岁、40~49岁、50~59岁、≥60岁各年龄组患者三种感染情况的总体分布差异无统计学意

                   例HPV16转阴,转阴时间1~39个月,中位数8个月,差异有统计学意义(U=6681, P<0.0001)。

                   结论:对年龄小于50岁,且合并其他高危型感染的HPV16感染患者,应积极行阴道镜活组织
                   病理检查。

                                 Clinical characteristics of 542 patients with HPV16 infection

                   【Abstract】  Objective To probe the clinical characteristics of patients with high-risk human
                   papillomavirus of human papillo- ma virus (HPV)16 infection. Methods A total of 542 patients
                   who were positive for HPV in HPV molecular test were selected from Beijing Jingmei Group

                   General Hospital from January 2013 to December 2021. The patients were analyzed for co-infection
                   with other HPV, infection in different age groups and negative conversion of HPV16. Results Of
                   the 542 patients, 330 ( 60.9%) were single positive, 173(31.9%) were complicated with other high-
                   risk infections, and 39(7.2%) were complicated with low-risk infections The prevalence rate of CIN

                   II and above in cervical biopsy with other high risk group (44. 2%) was higher than that in cervical
                   biopsy with low risk group (21.4%),    (χ = 4. 880, P= 0.027). There was no significant difference

                   in the overall distribution of the three types of infection among the age groups ≤29,30-39,40-49,50-
                   59 and ≥60, (χ  = 11.185, P = 0.191). There was no significant difference in the constituent ratio of

                   cervical biopsy between ≤29-year-old group and 30-49-year-old group (χ  = 3.369, P = 0.338). The

                   detec- tion rate of CIN III in 30-49 age group was higher than that in other age groups (χ  = 4. 566,

                   P = 0.033), and the detection rate of CIN was higher in under 50 age group (χ  = 10.313, P<0. 001).
                   Among the 183 follow-up patients, 134 cases turned negative for HPV16, with a median time of 16
                   months (2-75 months), and 152 cases turned negative for HPV16, with a median time of 1- 39
                   months (1-39 months), the median was 8 months (U=6681, P<0. 0001). Conclusions Clinically,
                   colposcopic biopsy should be performed actively for patients with HPV16 infection who are less
                   than 50 years old and complicated with other high-risk infections.
